I've gotten this as well. I'll try to reproduce it.
All I did was have 7.5MM rows being inserted in a while loop and one
failed with this exact message message ~175k inserts in so I'm not
quite sure what happened.
I'm running the same thing again but this time logging to disk instead
of echoing and quitting.

This has been reported by someone else too, and it seems to only happen on the first query.
I haven't been able to reproduce it since. What does it mean? Is it something to worry about?
It probably means the optimiser pipeline isn't initialised as you expect it to be.
